Instructions
Prior to application, qualified healthcare professionals (HCP's) should confirm or clarify coding and coverage with their respective local Medicare contractor, as each contractor may have differing formal or informal coding or coverage policies. HCP's should also obtain prior authorization from private payers or Medicaid, where applicable.
HCP's are responsible for accurate documentation of patient conditions and appropriate reporting of procedures and products.

Indications and Contraindications
KERAMATRIX products are indicated for the management of wounds including:
- Partial and full-thickness wounds
- Pressure ulcers
- Venous ulcers
- Chronic vascular ulcers
- Tunneled and/or undermined wounds Diabetic ulcers
- Trauma wounds (abrasions, lacerations, second-degree burns, skin tears)
- Draining wounds
- Surgical wounds (donor sites/grafts, post-Mohs' surgery, post-laser surgery, podiatric, wound dehiscence)
KERAMATRIX products are not indicated for third degree burns.
Please review the Local Coverage Determination (LCD) specific to the Medicare contractor that processes your claims, the medical policies of the private payers in your geographic area, and your state's Medicaid policies to confirm the indications that are covered.
